Why Do I Need an Occupational Therapy Report?
Occupational Therapy (OT) reports can have a major impact on your OT support journey or the provisions you’re entitled to through the NDIS. Reports like a Functional Capacity Assessment (FCA) or disability housing reports, are not just paperwork. They’re roadmaps that highlight how someone uniquely interact with the world and what support they might need to maximise independence. OT Reports delve into sensory processing, motor skills, and day-to-day independence, giving your support team the insights to make informed choices

An accurate, well-written, and client-centred Functional Capacity Assessment can make a huge difference in the level of funding that a participant receives. Our Functional Capacity Assessments are written collating a wealth of information including an Occupational Therapy functional review, appropriate standardised assessments, and other medical reports
We highlight where your exact needs lie and where the NDIS should direct funding. These assessments are invaluable for accessing the NDIS, plan reviews, changes in circumstance, or funding appeals. We understand the time-sensitive nature of these reports and offer a turnaround time of 10 business days
The NDIS offers a wealth of housing options including Supported Independent Living, Independent Living Options, and Specialist Disability Accommodation. A thorough, accurate, and well-written report can make the difference between being approved or rejected by the NDIS
Understanding the importance of these reports, our therapists pride themselves on writing client-centred, highly detailed reports that include the appropriate assessments and information to give you the greatest chance of success
The Sensory Profile (SP) provides a standard method for professionals to measure a child’s sensory processing abilities. This assessment is used to identify sensory processing patterns and to reveal how those patterns support and/or interfere with a person’s functional performance. There are four patterns of sensory processing, which include: seeker, avoider, sensor, & bystander
Please note that the SP interpretation and report will discuss the general features and processing patterns of a participant based on their scores. The SP report itself does not provide specific consumable recommendations or specific strategies to address behavioural concerns. It is highly recommended that an SP report be utilised by their ongoing treating Occupational Therapist who can implement, monitor, and adapt consumables and strategies
A tailored and precise prescription is key to enhancing an individual’s independence and quality of life. Our Assistive Technology Prescriptions are crafted with meticulous attention, focusing on selecting the most suitable technology solutions that align with the client’s unique needs and goals
We leverage a comprehensive evaluation process, encompassing functional assessments and client consultations, to recommend technologies that can significantly improve daily living. Whether it’s mobility aids, communication devices, or environmental controls, our goal is to empower clients by facilitating greater autonomy and participation in their desired activities
Supported Independent Living Assessments evaluate and determine the optimal level of support required for individuals to live as independently as possible within a shared living arrangement. These assessments are crucial for individuals who are seeking to transition to, or maintain, a supported living environment but require assistance with daily tasks. Through our evaluations, we identify the individual’s abilities, support needs, and preferences, focusing on enhancing their independence while ensuring their safety and well-being
These assessments detail the necessary supports that align with the individual’s goals, covering aspects such as personal care, community participation, and household management. These OT reports are particularly beneficial during transitions to new living situations, changes in support needs, or when seeking to optimise current support arrangements
The Individual Living Options Assessment is for those exploring personalised living arrangements outside of traditional group or supported living settings. This assessment is vital for individuals who desire a more autonomous living situation, tailored to their unique needs and preferences
We conduct a thorough evaluation to understand the individual’s capabilities, support requirements, and lifestyle aspirations, aiming to identify and facilitate the most suitable living arrangement. Whether it’s living alone with support, cohabitation with peers, or family-governed living, our assessment provides the groundwork for making informed decisions about the most fitting and sustainable living option. These OT reports are especially important when considering major life transitions, seeking increased independence, or when traditional living arrangements no longer meet the individual’s needs or preferences
Our Home Modification Assessments are designed to create safe, accessible, and supportive living environments for individuals with disabilities. By conducting an in-depth analysis of the client’s living space and their specific needs, we provide personalised recommendations for modifications that enhance mobility, function, and independence. Our assessments cover a broad range of modifications, from simple grab bar installations to more complex structural changes
Working closely with clients, families, and builders, we ensure that the proposed modifications not only meet the client’s current needs but are also adaptable for future requirements. With a focus on promoting independence and safety, our home modification solutions aim to transform living spaces into empowering environments that support the client’s lifestyle and activities
