Initial Assessments
Occupational Therapy Report
An Initial Assessment is the first step in occupational therapy. It gives your occupational therapist a clear understanding of a participant’s strengths, daily routines, and where support could help, so therapy or further reports can be planned around real goals

What is an Initial Assessment?
An Initial Assessment is a structured review of how a participant manages everyday activities, from self-care and routines through to mobility and participation. Your occupational therapist looks at what’s working well and where challenges come up, building a clear baseline to guide the support ahead. It’s often the starting point before ongoing OT sessions or a more detailed report
- Daily living skills and routines
- Physical, sensory, and cognitive functioning
- Strengths and areas of difficulty
- Goals that matter to the participant
- Recommended next steps for support
Our Initial Assessments go beyond simple screening – we use clinical observations, standardised assessment tools where appropriate, and collaborative discussions to build a thorough understanding of your unique situation. This ensures your therapy journey starts with a clear direction and measurable objectives
Initial Assessments play a crucial role in establishing effective occupational therapy support. These assessments are not just an intake process – they’re the foundation that shapes your entire therapy journey and ensures interventions are targeted to your specific needs. A thorough, well-structured Initial Assessment provides clarity by identifying:
- Your current abilities and skills across daily living activities
- Specific areas where you experience challenges or require support
- Your personal goals and what you want to achieve through therapy
- Environmental factors that support or hinder your independence
- Recommended interventions and therapy approaches for your situation
- A baseline for measuring progress throughout your therapy journey
