We’ll collect background information including medical reports, previous assessments, current support arrangements, and your goals for supported living

Our occupational therapist will conduct a thorough assessment of your functional abilities across self-care, domestic tasks, community participation, and social activities, observing you in relevant environments where possible

We analyse your support needs across different times of day and activities, determining the intensity and type of support required for you to live safely and work toward your goals
